Background: Oroantral communication is an iatrogenic complication following extraction of the maxillary posterior teeth (premolars and molars), due to the close relationship of the roots to the antrum and partially very thin maxillary sinus floor. Aim of Study: Evaluation of Buccal Pad of Fat versus Leukocyte Platelet Rich Fibrin (L-PRF) in the management of oroantral communication and periodontal surgeries. Materials and Methods: This study was carried out on twelve patients with complication of oroantral communication as well periodontal flap surgeries. They were divided into two groups, 6 patients in each. In Group I, the patients were managed with buccal fat pad technique. While in group II, the patients were treated with L PRF technique for OAC closure. Patients were followed up at 1,3,5,7 days post-operative then weekly for 4 weeks. Results: There was significant difference observed between the two groups considering pain, and swelling. Pain and swelling was more commonly observed in patients managed with buccal fat pad. However, no significant difference between both groups regarding infection during different follow up periods. No recurrence was noticed for the oroantral communication in all patients of both groups after 4 weeks postoperatively. Conclusion: Although both the techniques gave better results with no cases of recurrence and infection in both techniques but the leukocyte-platelet rich fibrin method was slightly better because pain and swelling was less in this technique as compared to buccal fat pad technique.
