Abstract

Orthodontic extractions remain a somewhat controversial topic. The removal of teeth should be considered as a therapeutic option in cases of insufficient facial harmony, space constraints, and instability in occlusion. The decision to extract is influenced by factors such as the severity and type of malocclusion, the patient’s age and morphological characteristics, dental history, oral hygiene, caries rates, and the quality of the teeth. Specific teeth may need to be extracted depending on the type of malocclusion. In certain scenarios, timely extractions can lead to spontaneous correction of the malocclusion. Extractions can involve various combinations of teeth. The indications and contraindications for these extractions are detailed, highlighting the advantages and disadvantages in different cases. Extractions have been shown to facilitate profile changes, aid in teeth alignment, and reduce lower facial height. Deciding whether or not to extract requires careful and thorough diagnostic evaluation.
